Spontaneous Retroperitoneal Hematoma Associated With Enoxaparin: Case Report

She had a history of mitral valve replacement operation 8 years ago and using 2 X 4000 IU enoxaparin injection treatment for tromboembolic prophylaxis. On the 3rd day of admission she was complaining about sweating, dispnea, abdominal and back pain. She had pulse rate of 140/minute, respiratory rate of 25/minute and a blood pressure of 60/30 mmHg. Abdominal tenderness and muscular defence were the physical examination findings. Haemoglobin was 3.32 g/dl, hematocrit of 8.63%, platelet count 92.5X103 /mm3 and INR of 2.5. Urgent abdominal tomography was obtained and retroperitoneal hematoma revealed measuring 25x13x10 cm in right retroperitoneal space. Anticoagulant therapy was discontinued and 12 units of blood transfusions, 10 units of fresh frozen plasma were administered within 2 days. After supportive treatment the patient’s vital signs stabilized, control haemoglobin was detected as 9.05 g/dl and INR was 1.5. Retroperitoneal bleeding is one of the most serious and rare complications of enoxaparin. Anticoagulant therapy receiving patients who have renal impairment, advanced age and comorbid diseases should be carefully observed due to potentially fatal complication.Keywords: Hemorrhage; retroperitoneal space; enoxaparinTurkiye Klinikleri J Cardiovasc Sci 2007, 19:186-189
